Basic Payroll Customers: Completing Form 941

Learn how to download and fill out Federal Form 941 for Quarterly federal tax withholding.

Available for: Basic Payroll customers and Full Service Payroll Customers who have not completed their payroll tax filing setup.

If you are a Full Service Payroll Customer who has completed the tax filing setup, you can find your 941, state withholding filings, SUTA filings, and local tax filings in your tax filing packet. Please see, “Viewing Your Company Tax Filing Packet,” for more information.

Definitions:

  • Form 941 (Employer’s Quarterly Federal Tax Return): The IRS form employers use to report federal income tax withheld from employee paychecks, as well as the employer and employee shares of Social Security and Medicare taxes (FICA). Form 941 is filed with the IRS four times per year.

  • Basic Payroll: The Patriot Payroll plan where you calculate payroll and manage employee pay yourself, but you are responsible for depositing and filing your own payroll taxes. Patriot does not file or pay taxes on your behalf under this plan.

  • Full Service Payroll: The Patriot Payroll plan where Patriot handles payroll tax payments and filings for you.

  • Deposit schedule: The IRS-assigned schedule (monthly or semi-weekly) that determines how often you must deposit your federal payroll taxes. Your deposit schedule is based on the total taxes you reported during the lookback period.

  • Lookback period: The 12-month window the IRS uses to determine your deposit schedule for the calendar year — specifically, July 1 of the second preceding calendar year through June 30 of the preceding calendar year. For example, the lookback period for 2026 is July 1, 2024 through June 30, 2025.

  • EFTPS (Electronic Federal Tax Payment System): The free IRS system used to make federal tax deposits electronically. As of the 2026 revision of Form 941, all federal tax deposits must be made electronically, either through EFTPS, IRS Direct Pay, your IRS business tax account, or arrangements made through your financial institution.


Form 941 Deadlines

Quarter 1 – 4/30
Quarter 2 – 7/31
Quarter 3 – 10/31
Quarter 4 – 1/31

💡When the end of the month falls on Saturday, Sunday the due date is delayed until the next business day.


How to Download Your Pre-filled Form 941


Patriot provides Basic Payroll customers with a partially pre-filled Form 941 PDF based on your payroll data for the quarter. To download it:

  1. Go to Reports > Payroll Tax Reports > 941s

  2. Select the year and quarter for the form you want to download.

  3. Click Download. A partially completed Form 941 will download to your computer.

  4. You can also download instructions to complete your Form 941.

  5. Click the downloaded file to display and complete the form.

    1. Unless you have a full version of Adobe, you cannot save the information typed into the PDF form.

    2. You can type the remaining fields and then print the form for your records, or handwrite the missing information on the printed form.

If you need to print only specific pages of Form 941 (e.g., pages 4–5), open the filing packet, click Print, and specify the desired page range.

What Patriot Pre-Fills on the 941 for Basic Payroll Customers

Based on the current Form 941 (Rev. March 2026), Patriot pre-fills the following fields from your payroll data:

  • Employer identification number (EIN), business name, trade name, and address

  • The quarter checkbox at the top of the form

  • Line 1: Number of employees who received wages for the pay period that includes March 12, June 12, September 12, or December 12

  • Line 2: Wages, tips, and other compensation

  • Line 3: Federal income tax withheld

  • Line 5a and 5c: Taxable Social Security wages and taxable Medicare wages and tips, including the calculated tax amounts in Column 2

  • Line 5e: Total Social Security and Medicare taxes

  • Line 6: Total taxes before adjustments

  • Line 10: Total taxes after adjustments

  • Line 12: Total taxes after adjustments and nonrefundable credits

  • Your printed name and daytime phone number in Part 5


What Basic Payroll Customers Must Complete on Form 941

Because Patriot does not deposit your taxes under the Basic Payroll plan, several fields require information only you have:

  • Line 13, Total deposits for this quarter: Enter the total federal tax deposits you made with the IRS for the quarter, including any overpayment applied from a prior quarter or from a Form 941-X, 941-X (PR), or 944-X filed in the current quarter.

  • Line 14 or Line 15a: Once you've entered your deposit total on Line 13, compare it to Line 12.

    • If Line 12 is more than Line 13, enter the difference on Line 14 as your balance due.

    • If Line 13 is more than Line 12, enter the difference on Line 15a as your overpayment.

    • Never make an entry on both lines.

  • Line 15b–15e, refund or credit election: If Line 15a shows an overpayment, check whether you want it applied to your next return or refunded on Line 15b.

    • As of the 2026 revision, the IRS issues Form 941 refunds by direct deposit. If you want your refund direct deposited, complete your routing number (Line 15c), account type (Line 15d), and account number (Line 15e). If you leave these blank, the IRS will send a check instead.

  • Part 2, Line 16 – deposit schedule and tax liability: Indicate whether you're a monthly or semiweekly schedule depositor for the quarter.

    • Your deposit schedule for the calendar year is determined by adding up the total taxes you reported on Line 12 of Form 941 across all four quarters in your lookback period.

      • If that total is $50,000 or less, you're a monthly schedule depositor for the entire year. If it's more than $50,000, you're a semiweekly schedule depositor for the entire year.

      • This determination is made before the year starts and doesn't change quarter to quarter based on current-year activity.

    • If you're a monthly schedule depositor, enter your tax liability for each month directly on Form 941. The total must equal Line 12.

    • If you're a semiweekly schedule depositor, you must also complete and attach Schedule B (Form 941). See Do I Need to Fill Out Schedule B? and our related article for step-by-step instructions on completing it.

  • Part 3: Check the boxes only if your business closed during the quarter or you're a seasonal employer.

  • Part 4: Complete only if you want to authorize a third-party designee to discuss the return with the IRS.

  • Part 5, Sign Here: Sign and date the return yourself. Patriot pre-fills your printed name and phone number, but the signature line and date must be completed by you.

  • Form 941-V (Payment Voucher): If you're paying a balance due by check or money order, Patriot pre-fills your EIN, business name, address, and quarter. You must still enter the payment amount and detach the voucher to mail with your payment.

Note: The Aggregate Return Filers Only section near the top of the form (Section 3504 Agent, CPEO, or Other Third Party) applies only to filers submitting an aggregate return on behalf of multiple employers. Basic Payroll customers filing their own return should leave this section blank.


Do I Need to Fill Out Schedule B?

You need to complete Schedule B (Form 941) if you're a semiweekly schedule depositor for any part of the quarter.

  • Your lookback period determines your default schedule. Your deposit schedule for the calendar year is based on the total taxes you reported on Line 12 of Form 941 across all four quarters in your lookback period (see Definitions, above). If that total is $50,000 or less, you're a monthly schedule depositor for the entire year and don't need Schedule B. If it's more than $50,000, you're a semiweekly schedule depositor for the entire year and do need Schedule B.

  • The $100,000 next-day deposit threshold can also trigger it mid-quarter. Accumulating a $100,000 tax liability on any single day triggers two separate things: you must deposit that amount by the next business day, and you also become a semiweekly schedule depositor for the rest of the current calendar year and all of the following calendar year, even if your lookback period made you a monthly depositor. Because Schedule B is required for any quarter in which you were a semiweekly depositor for any part of it, you must complete Schedule B for the entire quarter in that case, not just the days on or after you crossed the threshold.

  • New employers: if you didn't exist for the full lookback period, any quarter you didn't exist is treated as zero tax liability. Most new employers start out as monthly schedule depositors unless the $100,000 next-day rule applies during the year.


Additional Tips

  • To verify payroll amounts for a quarter, use Reports > Payroll Reports > Payroll Details, set the date range, keep all employees, locations and sources checked and and group by pay date.

Troubleshooting and FAQs

  1. Missing Quarters in Dropdown: If Q1 does not appear, it may be because no payrolls were processed with Q1 pay dates. Without payroll data, the form will not be generated.

  2. Employee Termination: Terminating an employee does not affect quarterly tax filings, including Form 941.

  3. Employee Count on the 941: The IRS wants a report of the number of employees who worked during the date of

    1. March 12, June 12, Sept 12, Dec 12

    2. This is the only time the PAY PERIOD and not the PAY DATE matters to the IRS. For instance, a company has paid 4 employees in the month of March. Three of the employees were paid with a pay period of 3/1- 3/13. A total of 4 employees for the pay period of 3/14-3/29. Because only three employees worked on or through 3/12, the 941 would report "3" in line one of the 941.


What if I am an Agricultural Business?

Patriot does not supply or file IRS form 943 for agricultural businesses. If you need form 943, we suggest you use our basic payroll and manually fill out IRS form 943 on your own using our payroll reports.
